The purpose of this form is to allow users the opportunity to request additional information about IMEDECS services. IMEDECS is not responsible for protection or privacy of electronic mail or other information transferred through the Internet or any other network provider you may utilize. The Guest Book is not intended to be used by patients for contacting IMEDECS regarding a case review. Personal health information should not be entered into the Guest Book.

The Site is not intended or designed to attract children under the age of 13. We do not collect personally identifiable information from any person we actually know is a child under the age of 13.
