Preparation of Records

  • 1. In preparing the records, please organize the documents in sections (for example health plan summary, appeal/denial information, health plan language, criteria, medical records, etc.) in chronological order from the earliest date to the latest date. See attached Checklist of Materials to Include for Case Review. Please remove unnecessary materials such as bills, past progress notes not relevant to the questions being asked and any other documents not applicable to the review. Inclusion of unnecessary materials increases the physician reviewers' time and ultimately the cost of the review.
  • 2. IMEDECS will manage the remainder of the review process which includes:
    • Distribution of medical records to the expert reviewers
    • Monitoring the timely return of reports
    • Providing a quality check reviewing the content for accuracy and to assure that questions posed have been answered clearly
    • Forwarding reports
  • 3. IMEDECS must facilitate all communication between the expert reviewers and the health plan. If you have any questions or concerns regarding the written reports, or if you wish to provide additional information for the reviewers to consider, please call IMEDECS and we will assist you. We require that we monitor all telephone interactions between our reviewers and attending physicians, patients and health plan representatives.
